junkairenraku ?!

Un cop just went by my house, he asked me to fill out a form, conveniently enough the form is in english. According to the form this is something supposedly used to improve communications between citizens and the police and to prevent car accidents.
The thing is, in four yeas living here it's the first time i see anything like it. The form mention this is a standard procedure called junkairenraku. The other thing is that the form asks for very precise information like immigration registration number, date of birth, current job, duration of visa etc...
It also has a field for me to pass a message to the police, basically what would i want the police to be concern about in the neighborhood.
Does anybody know what this is about ? Is it really standard ?

Happened to me in my last place. When we moved to the new place we voluntarily went into the nearest Koban to fill one out. Just to stop the plod from calling at the front door.

Might be kinda useful if "THE BIG ONE HITS" (TM) to make sure that the plod has confirmed the last place to help survivors.

Or so he can confirm the first place to round up foreign dissidents and saboteurs before they start fires and throw poison in the wells after "THE BIG ONE HITS".

at the last place i lived - in koto ku - a cop came by sometime after i moved in and asked me to fill out this form. i think it was this form. i assumed it was standard and i thought it was actually kind of a nice idea. i of course thought it was also more than likely used more for profiling and keeping track of dangerous elements like myself.
anyhow, no big deal.
i moved last march to chiyoda ku. i am near a large police station and i popped by soon after my move. i stated that i had just moved in to the neighborhood but the policeman kind of just dismissed me. not rude, but he (or she, i forget) had really nothing for me to do.
i also asked where the kuyakusho is and the cop asked around for me and showed me on a map. (i couldn't believe it was such a tough question...everyone knows where the kuyakusho is!)
