I went to one of the big hospitals in Tokyo with English speaking doctors thinking it would be better and they gave me that first-aid kit cloth sling. After about a day of using it my neck hurt worse than my shoulder. I called around and found a clinic that sold slings. The doctor told me the other place probably didn't offer me one because they aren't covered by national insurance. I think it cost me 3000 yen and change.
When the doc brought the sling out is was too small even though it was adult one size fits all (he said it was unfortunately Japanese one size fits all). I'm only 173 cm tall and at the time weighed 78 kg (5'8", 172 lbs to my fellow Mercans). Not exactly skinny but not an unbelievable size for Japan. It was just big enough I could use it and it was better than the giant white handkerchief but it was still uncomfortable.
By the way, did they X-ray your good shoulder also to compare with the injured one? I've had that happen to me twice here. Fucking witch doctors.
I've noticed they still have the old school plaster casts in Japan too. I don't think I've seen one of those in the US since the 80s.
