Most major hospitals in Japan now work on a patient based request program. If you want to know if you have cancer, they will tell you. Also, you can either have them withhold that information from your family, or inform your family. You even have an option of having them inform your family, but not tell you, yourself.
I don't know if this holds true to the small cut and slash type of clinics, but it seems to be at all of the major facilities.
