JPLT Applcation form - where can I find it?

JPLT Applcation form - where can I find it?

Yes I know the deadline is tomorrow...I figured I could just walk over to Kinokuniya and pick up the form but I was there today and they didn't have any. Instead they told me to call a phone number (BonJinSha). However the number doesn't work, I just get the occupied tone all the time. So what next? the JPLT is not very helpful as they give a list of places where you can get the form but it is in japanese...

The study support home page simply reflects:

8-9. Japanese Proficiency Language Test

Till 2000, most of the universities or vocational schools asked applicants to submit their score of Japanese Proficiency Language Test level 1 or 2. The number of schools requiring applicants to submit score of JPLT decreased after the debut of Examination for Japanese University Admission for International Students (EJU) from 2001. However, there are still lots of schools require the score of JPLT.
JPLT will be held in December every year. Application forms are sold in the major book store over Japan. It?fs price is 500yen and test fee costs 5,460 yen. The deadline for application is around 10th September....."

I would just start hiking! Kinokuniya ain't the only Honya in town and most of them should be willing to help an FG (honya's for some reason, are that way). Hopefully, you not in the inaka somewhere.
