You have several options:

    Before embarking on an "adventure" vacation, I have bought insurance at one of flight insurance kiosks at Narita Airport. Just ask at the airport information desk where the flight insurance kiosks are located...or use kiosk locations and phone numbers are here: http://www.narita-airport.jp/en/guide/service/list/svc_02.html .
    The high-risk travel insurance by AIU that I purchased at the airport cost me over 4,000 yen, ouch! http://www.aiu.co.jp/travel/
    On another high-risk vacation, I bought a cheap "rider" on my renter's insurance (AXA) for 2,000 yen for a 30-day vacation in the jungles of the Yucatan.
    The best deal I found for my needs, was to upgrade my AMEX card to a premium Gold/Platinum/Black level. That covers everything travel-wise (medical coverage, emergency evacuation, VIP waiting room, rental car insurance, lost luggage insurance, etc. ) for 4000yen/year. This has turned out the cheapest deal for me because I travel a lot. It may be possible to get the same kind of a "free" insurance deal from a Japanese gold card issuer such as Master Card.

That doesn't seem too bad.

When I went back to the US last month for 3 days the travel insurance I purchased was about $80.00. This covered everything though.

4,000 yen isn't so bad I guess:(

Earlier this this year, I went to this site which I've used before:

Compared to the policy my Japanese gf had bought for us through AIU for a trip back to the US a few years ago, this was much more reasonable. Plus, the policies and procedures being in English were a lot more easier to understand.
