Not just Tohoku (although I did have that experience here recently).

It can happen anywhere there's a crowd. In Tokyo I've seen it happen at the old Kabuki-za and a couple of other theaters. During intermission, of course, when there's a sudden "surge" of toilet usage. It's usually older women that do it, and once one does it the rest follow suit.
