Roppongi Hills complex starts removing all revolving doors
TOKYO Aug. 26 Kyodo -The owner of Tokyo's Roppongi Hills high-rise complex began work at midnight Wednesday to remove all revolving doors, following a fatal accident in March.
The upscale complex in the Roppongi district houses shops, restaurants, offices, a hotel and a museum. It has nine large-size automatic revolving doors including the one that killed Ryo Mizokawa, a 6-year-old boy from Osaka, as well as 40 small revolving doors.

Dude, we've discussed this before; the doors were defective.. Meaning they didn't work properly.. They didn't stop or start when they were supposed to.. They done be broken..

I witnessed it, experienced, and even mentioned something to the gaurds before this incident; The most fatal of the 63, or so incidents! How can you still place all the blame on a careless mother?

That's like saying if an elevator breaks, crashes to the ground, kills everyone inside, after people had previously warned that the elevator didn't work properly. Then blaming the people riding the elevator because they didn't take the stairs..

Not everyone injured in these doors were children..

MDN: 'Domino' pile-up on escalator leaves boy seriously injured
OSAKA -- A young boy was seriously injured after more than 10 schoolchildren fell over like dominos on an escalator at a municipal education facility here Wednesday afternoon, police said.

At around 1:20 p.m., nearly 15 schoolchildren who were visiting Kids Plaza Osaka in the Ogimachi district of Kita-ku, Osaka, suddenly fell over like dominos while riding an escalator from the third to second floor, local police said.

Tokyo Metropolitan Government: Responses to the key tasks facing the Tokyo Metropolis

An issue of growing public concern is that of accidents which involve familiar features of everyday urban life. This was highlighted in the wake of a recent fatal accident involving automatic revolving doors.

Following the tragedy at the Roppongi Hills building, TMG conducted an urgent screening of last year's emergency accident data, which involved approximately 800,000 cases. The investigation identified a high frequency of accidents at escalators, and various discussions are now taking place to further deliberate on the issue. We will continue to gather and analyze information obtained in emergency operations, so as to widely identify and publicize the potential hidden dangers in urban life, and prevent future accidents.

Approximately 40% of all automatic revolving doors in Japan are concentrated in Tokyo, with around 70% of accidents involving senior citizens and small children. Determined to prevent a recurrence of the Roppongi Hills tragedy, we have proposed a revision to Tokyo's construction safety ordinance in this regular session of the Assembly. The amendment will set an upper limit for the revolving speed of such doors, and obligate building owners who install them to adopt enhanced safety measures.

An amendment to the fire prevention ordinance is also proposed in order to reinforce fire safety measures at underground railway stations. The proposed changes for ensuring the safety of Tokyo citizens include mandating the installation of sprinklers at all underground levels of station structures, as well as shutter doors with a two-step closing mechanism, and evacuation route signs which illuminate automatically in the event of a power blackout.

Asahi: Boy's revolving-door death snares execs
Suspected of professional negligence resulting in the death of a 6-year-old boy, about seven executives from the Mori Building Co. and revolving-door maker Sanwa Tajima Corp. will be referred to prosecutors, sources said Tuesday. The boy was killed by a revolving door at the tony Roppongi Hills complex, managed by Mori Building, in Tokyo's Minato Ward last March. Tokyo police say such an accident was foreseeable and that the two companies failed to take timely steps to prevent it. In fact, 32 mishaps involving revolving doors had occurred between the opening of the complex in April 2003 and the March 26, 2004, fatal accident.
The two company presidents were also questioned, but uncertainty over their understanding of the risks makes it unlikely they will be referred to prosecutors, sources said...Others had been injured prior to the fatal accident. On Dec. 7, 2003, a 6-year-old girl suffered ear and leg injuries when she was caught in a similar fashion in another revolving door at the complex. Two days after that incident, Mori Building and Sanwa Tajima officials discussed ways to put a stop to the accidents. They decided to erect barriers to prevent visitors from rushing into the doors as well as improve safety sensors that detect people and objects.
Because the December 2003 incident was similar in nature to the fatal one, police apparently decided the officials who attended the meeting must have been aware of the dangers the revolving doors posed and should have foreseen another grave accident, sources said. Police suspect their failure to take action resulted in the fatal accident the following March. Mori Building President Minoru Mori, 70, was not present at the meeting to discuss preventive measures after the girl's accident. However, it is now known that he was briefed on the accident and the preventive steps discussed in the meeting with Sanwa Tajima officials.
The official who briefed the president, a manager in charge of the complex, did not have an appointment but instead accosted him in a corridor on Dec. 24, about two weeks after the girl's accident. They stood while talking, sources said. Before walking away, the president is reported to have said "go ahead immediately" with the preventive measures. That informal meeting is not on record, sources said. Mori told police he had "no recollection of having received any report on the door accident that occurred at the Hills."
Because the report was made so hurriedly, police apparently decided Mori was not in a position to comprehend the risks posed by the complex's revolving doors and the need for preventive measures, sources said. The president of Sanwa Tajima was reportedly never informed of the matter.

NAGOYA -- Some 13 high school students were injured when they fell like dominos on an escalator at a local subway station after one of them stopped when his bag was caught between steps, police said. The high school student had his bag caught between steps on the escalator at Shiogamaguchi Station on the subway Tsurumai Line in Nagoya at about 8:20 a.m. on Sunday. When he tried to pull his bag free, he was hit by other students following him, resulting in 13 high school students falling, officers said.

Three execs charged over Roppongi Hills revolving-door death

Tokyo prosecutors indicted three former company executives Wednesday in connection with the death of a 6-year-old boy who was crushed in an automatic revolving door at the Roppongi Hills complex last March.
The Tokyo District Public Prosecutor's Office brought charges of professional negligence resulting in the death of Ryo Mizokawa against 62-year-old Yuzo Tada, a former managing director of complex operator Mori Building Co., 48-year-old Yukihiro Koyama, former head of the Roppongi Hills management operations room, and Hisanobu Kubo, 61, a former director of Sanwa Tajima Corp., the door's manufacturer.

The Metropolitan Police Department had sent the prosecutor's office cases against six executives, but only three were indicted.

According to prosecutors, a girl had been seriously injured in the same type of revolving door at another location in Roppongi Hills about four months before the fatal incident....more...

A cable supporting an elevator in the 54-story Mori Tower in Tokyo's Roppongi Hills district snapped and five other elevators were damaged when last year's Niigata Chuetsu earthquake rocked wide areas of eastern Japan, it has been learned.
It is thought that the wire cable snapped because sensors failed to stop the elevator when the earthquake struck....more....

Mori Building ex-execs convicted over fatal revolving door accident

TOKYO — The Tokyo District Court sentenced two former officials of Mori Building Co to 10 months in prison, suspended for three years, for professional negligence over the death of a 6-year-old boy in an automatic revolving door at Tokyo's Roppongi Hills complex in March last year.

Presiding Judge Hironobu Murakami said the defendants could have foreseen that such an accident would happen because a number of similar accidents had taken place at automatic revolving doors made by Sanwa Tajima, including ones at Roppongi Hills. The boy, Ryo Mizokawa from Suita, Osaka Prefecture, was killed when his head was crushed between the revolving door and the door frame as he rushed ahead of his mother into an entrance of the Mori Tower building at the complex on March 26, 2004.

While I can`t agree with Asskissinger`s attitude in toto, this whole thread which I only read today has got me scratching my head over just why so many Japanese have to behave like morons.

Why aren`t kids everywhere falling from escalators, slipping backwards like dominoes, getting their heads smashed in from admittedly dangerous doors? Yep, it does happen in other countries but less, and I`ve never seen such negligence re letting your kids behave like shits running around in public places, including dangerous ones.

Everyday I see Japanese jerks, male and female, old and young watch passively while some brat of a kid runs around on escalators, runs through shops etc, on busy roads, parks him/herself in front of glass doors in shopping areas (both manual doors and automatic) and all the while his ugly ass dwarf of a grandmother/grandfather, empty headed mother, couldn`t give a fuck father do jack.

Seeing this so often makes me wonder about the natural intelligence of the members of inbred societies like Japan. Yeah, their maths at elementary school etc is better than that of kids in America, but as far as actually been able to reason and think for themselves, Japanese of all age groups rank in the moron level of the intelligence category.

Part of it is the peasant mentality. Yep, that`s the conclusion I`ve come to. Not being able to think for yourself, excessive reliance on others` opinions no matter how misguided, unformed and basically fucked. An oppressive group mentality, an ijime culture. Despite all the high tech goodies here etc most Japanese are serfs - their society is feudal and they haven`t moved from the peasant cast of mind. Losers. Take responsibility for those fucking brats you produce.
