Yeah, looks fake to me:

- the old lady in the kimono didn't have a heart attack and die

- with the couple who were sleeping with their heads down, he looked kinda surprised but she was laughing way too much

- in the pachinko parlour only the guy carrying the balls freaked out. You'd think everyone nearby would at least jump.

Still pretty funny though! Funnier until I started looking to see if it was fake...
